What is Mounjaro and how does it work?
Mounjaro (tirzepatide) comes in a pre-filled pen with four fixed weekly doses. It activates both GIP and GLP-1 receptors — a dual action that slows stomach emptying, reduces appetite and stabilises blood sugar. It works best alongside healthy eating, exercise and ongoing medical support.
How Mounjaro works
By mimicking two gut hormones, Mounjaro reduces hunger between meals and 'food noise', while smaller meals bring more fullness.
How long before it works?
Mounjaro works from the first dose. Most people notice reduced appetite within a week; visible weight change usually becomes clear around month three.

In the head-to-head SURMOUNT-5 study, participants on the maximum tolerated tirzepatide dose lost an average of 20.2% of their body weight versus 13.7% with semaglutide.

Understanding side effects
Most side effects relate to digestion and occur early in treatment or after a dose increase. For most people they're mild to moderate and ease within the first 20 weeks.
Digestive issues
Nausea, diarrhoea, vomiting, constipation, indigestion, bloating and burping are the most common effects, usually strongest in the first few weeks and after each dose increase.
Other common side effects
Dizziness, low blood pressure, tiredness, injection site reactions, rash and temporary hair loss (more related to rapid weight loss than the medicine itself).
Dehydration
Persistent nausea, vomiting or diarrhoea can lead to dehydration, which can affect kidney function. Drink fluids regularly and tell your doctor if symptoms are severe or ongoing.
Less common but serious
Gallbladder problems, acute pancreatitis, increased heart rate, altered taste and menstrual changes are uncommon. Seek help immediately for severe abdominal pain with vomiting.
Eligibility

Is Mounjaro right for me?
- Adults with a BMI of 30 or higher
- Adults with a BMI of 27 or higher plus a weight-related condition such as high blood pressure, high cholesterol, heart disease, prediabetes or sleep apnoea
- Lower BMI thresholds may apply for people of South Asian, Chinese, other Asian, Middle Eastern, Black or African-Caribbean background
- People able to commit to lifestyle changes alongside treatment
Who shouldn't use Mounjaro?
- Under 18 or over 85
- Pregnant or breastfeeding
- Allergic to tirzepatide or any of the pen's ingredients
- Personal or family history of medullary thyroid carcinoma, or MEN2
- Severe kidney or liver problems, pancreatitis, gallstones or severe gastrointestinal conditions
- Currently taking other blood sugar-lowering medicines (discuss this with your doctor first)

What to expect over time
Most people reach 15–20% weight loss in the first year. Some reach 20–25% over 1.5–2 years at higher doses. Three-year data shows an average maximum loss of around 23%.

Taking it and dosing
Mounjaro dosage
Treatment starts at 2.5mg per week and is gradually increased every 4 weeks until you reach the dose that works best for you. Not everyone needs the highest dose.

How to use the Mounjaro injection
Use Mounjaro once a week, on the same day if possible, at any time of day.
Missed doses
- Within 4 days of the missed dose — take it as soon as possible
- More than 4 days — skip the missed dose entirely
- Take your next dose on your usual day
- Never take a double dose
Injection sites
- Stomach, at least 5 cm from the belly button
- Thigh
- Upper arm
- Rotate the injection site each week
How to inject
- Wash your hands and check the pen and expiry date
- Attach a new needle and prime the pen
- Dial your prescribed dose and listen for the clicks
- Inject, hold, and dispose of the needle in a sharps container

Don't use Mounjaro if you
- Are allergic to tirzepatide or any of the pen's ingredients
- Have a personal or family history of medullary thyroid carcinoma
- Have Multiple Endocrine Neoplasia syndrome type 2
- Are pregnant or breastfeeding

- Have a history of pancreatitis
- Have a severe gastrointestinal condition
- Have kidney problems
- Have diabetic retinopathy
Contraindications and interactions
- Insulin, sulfonylureas and meglitinides increase the risk of low blood sugar
- Other GLP-1 medicines should not be combined
- Absorption of oral hormone therapy may be reduced — a patch or gel may be more suitable
- Alcohol and some supplements can affect blood sugar and side effects
Mounjaro and contraception
Mounjaro slows stomach emptying, which can reduce absorption of the contraceptive pill. Use a barrier method or a non-oral method for 4 weeks after starting and after each dose increase.
When to seek urgent help
- Severe abdominal pain radiating to the back
- Persistent vomiting
- Severe upper abdominal pain with fever
- Signs of a severe allergic reaction

Storage & travel
Storing your Mounjaro pen
Store unopened pens in the fridge at 2–8°C, in the original packaging, and don't freeze them. Once in use, a pen can be kept below 30°C for up to 30 days.
How we deliver your Mounjaro
Mounjaro is a cold-chain medicine. We ship it in validated temperature-controlled packaging with cool packs that maintain fridge conditions for at least 48 hours.
Can I travel with Mounjaro?
Yes. Use an insulated travel bag with cool packs and a barrier between the cool pack and the pen. Carry it in your hand luggage — the hold can freeze and damage the medicine.

You may need a letter for airport security. Contact us and your doctor will send a PDF copy to print and carry with you.
Stopping, restarting and switching
Talk to your doctor before changing your treatment. A clear plan gives you the best chance of maintaining your results.
Can you stop suddenly?
Yes — tirzepatide doesn't cause physical withdrawal, so you can stop without tapering. The medicine leaves your body within a few weeks.
What happens after stopping?
Appetite and cravings often return to pre-treatment levels, and some or all of the weight lost can return. How you stop and how consistent your habits are both play a role.
Restarting Mounjaro
- Within 3 weeks — restart at your last dose, or build up further if it was well tolerated
- After more than 3 weeks — you may need to restart at 2.5mg and build up again
Switching to or from Wegovy
- Leave one week between your last Wegovy dose and your first Mounjaro dose, starting at 2.5mg
- Switching to Wegovy also requires a one-week gap; your doctor will advise the starting dose.
